FMMD 220 - INTERPROFESSIONAL CARE MANAGEMENT[40 Hours] The Patient Centered Medical Home (PCMH) practicum will incorporate the concepts of practice in a patient centered medical home. These concepts have been established in the Joint Principles for the Medical Education of Physicians as Preparation for Practice in the PCMH which was published by the American Academy of Family Physicians, the American Academy of Pediatrics, the American College of Physicians, and the American Osteopathic Association. The goals of this practicum are 1) to engage interprofessional learners in management of high risk patients (diabetics); 2) to learn interprofessional teamwork focused on meeting target indicators for this population; 3) to promote patient self-management of their chronic condition; 4) to introduce activities in practice and learning that result in practice improvement. Practicum experiences will include team meetings to discuss and monitor the progress of selected patients from the diabetes registry and to practice improvement projects.
